The practice is welcoming new patients who live in the catchment area which is approximately five miles in radius from WR9 8RD and highlighted on the map below:

The GMS1 registration form is available by clicking on the link below or please hand in your completed medical card if you have one. Unfortunately we cannot register patients by telephone but the completed forms can be emailed to email@example.com
The transfer of your medical records is quicker if you provide your NHS number. It can be found using this link https://www.nhs.uk/nhs-services/online-services/find-nhs-number/
To register, please complete both the registration form and questionnaire and return both to the practice. These forms can be emailed to the practice on firstname.lastname@example.org
Do I need ID to register?
You do not have to provide ID when registering with a GP, but it is helpful to do so.
All patients at the practice, including children, are allocated a named accountable GP, who is responsible for their overall care at the practice - this is referred to as your 'Usual GP'. If you are not aware of your usual GP please do check with any member of staff at any time.
We encourage you to see your usual GP where possible, particularly for ongoing problems, so we can provide you with better continuity of care. If he/she is fully booked, you can ask for an appointment with another doctor. We will make reasonable efforts to accommodate requests to change Usual GP.
When you have registered
You will be offered an appointment for a new patient health check with one of our Health Care Assistants or nurses. This involves taking a record of your medical history and general health, weight, blood pressure and in some cases urine/blood test. This service has been postponed due to COVID19.

The "Friends and Family" Test
All GP Surgeries in England are now giving patients the opportunity to comment on the services they receive though a simple new questionnaire.
The Spa Medical Practice is trying to make it as easy as possible to provide us with feedback. All replies are confidential, and you can also say why you have given the feedback you have.
All patients for whom we have current mobile phone numbers will get an automatic feedback text message after attending a surgery appointment.
Paper forms are also available on the front desk - a dedicated mailbox is available for returning them.
Each month the replies are counted and fed back to the NHS. The surgery also scrutinises them and looks carefully at any other feedback given.

Change of Details
Please inform the practice immediately if you change your name, telephone number or move to a new address. If you wish to change your name please bring a copy of any relevant certification for example marriage or deed poll. If you move outside the practice boundary you may be asked to register at a practice closer to your new home.
It is essential that we keep our records up to date in case of an emergency or hospital referral.

In order to maintain the quality of care we provide, we have practice boundary which will limit the distance that we can travel for home visits.
The name of you nearest relative or carer is useful if you live alone.
You may be asked if you either care for some or you have a carer. If you are the main carer for a relative/friend and would like to be officially recognised please inform the practice.
